-Learn about strategies to improve and enhance Advance Care Planning and End of Life Care.
-Determine gaps in advance care delivery and where enhancements can be made within the hospital and comunity setting.
-Utilize and implement key processes to ensure the POLST form is used effectively to support End of Life Care.
