Participating hospitals have agreed to submit their readmission data into a data portal on the ARC website. In ARC 1 the same measures were collected for all facilities.

In ARC 2 hospitals have the ability to add any other measure their hospital is using to track readmissions. In the data portal hospitals can see their own data over time as well as the collaborative over time. Hospitals can also compare their data to the rest of the collaborative for any time period(s).

Since the inception of the ARC program many of our engaged partners have become content and implementation experts in the area of care transitions improvement.

We think it is important to utilize the knowledge and skills of our partners to support the successful operationalization of improvement efforts at all of our participating hospitals.

Members of Speakers Bureau will participate in speaking opportunities to support the efforts of the collaborative. This may include participation at local or national events.
